Download the Repair Order form using the link at the top of this page labeled Download Form.
Open the form and fill in all applicable fields, including the signature field using the signature tool. You may need to download Adobe Acrobat Reader or a similar PDF reader to complete the form.
Submit the completed form using the Choose File button below. On iOS you may need to save or convert the PDF to an image.
Complete the required fields above, and the reCAPTCHA field, then click Submit.You can also submit the form by emailing it to ultimatebenz@bellsouth.net or bringing it with you to your appointment.
